Updated March 15, 2022: Added the link to the application launched by Cuba on November 15, 2021 which brings together the three forms that we are given on the plane (health declaration, visa and customs).
See the link to download the tool and understand how to fill it in this article: https://www.passionvaradero.com/post/electronic-form-for-entering-cuba-obligatory-from-january-1-20222
Of course, you need a valid passport .
Since November 7, you also need proof of Covid insurance as required by Cuba.
You must also download the vaccination passport valid outside Quebec and why not print a copy as well. It includes the logos of the province and Canada.
In Quebec, you download the passport from the same place as the one you use for restaurants, but you must choose the one for use in the context of travel outside Quebec, which is located further down the page.
You must also download the ArriveCAN app (it is strongly recommended to do this before your departure) or create an online account.

On the plane, you will need to complete three forms:
visa: French is on the third line - both sections must be completed without crossing out
Health Declaration: Spanish and English only
Customs: Spanish and English only
For people who are not comfortable with Spanish and English, I have made versions with French translations/information. I have also placed the blank version of the health declaration that you can print and fill out before your departure.
Have a good trip !
